What to Bring to Your First Visit

To make the most of your first visit, we recommend that you print, fill out and bring the New Patient Information Forms. You will also need to bring the insurance card, a valid ID, and all that apply to your child: updated immunization records, old medical records, hospital discharge records, and any special forms that you may need us to complete. Also, make sure that you have designated Dr. Pizarro Lopez as your child's Primary Care Provider by calling the number on the back of your card.

We accept most Commercial Insurances, as well as Simply Healthcare and Sunshine Medicaid.
